Location
Mindflayer's spawn locations are the Tier 5 dungeons with the purple orbs with a Blue Squid Icon on the map and the red dungeon glow on the mini-map.
Strategy
You can cheese the mindflayer by getting him stuck at an entrance and shooting him with any bow or staff.
